  • Name of organization
  • Contact information
  • Date and time of presentation (first and second choices)
  • Address of facility where presentation will commence
  • Estimated participants (minimum of 20 participants)
  • Bilingual or hearing-impaired interpreter
  • Topic of interest (see above)
